Referring Doctors We welcome referrals.Dentists, dental specialists, family doctors, emergency physicians and other medical professionals are a common source of new patients to our practice. We look forward to meeting your patients and consider it a privilege to assist with their treatment. We receive many referrals from previous patients and family members who were happy with the care provided by our doctors and staff. You may refer patients to our office by filling out our online Patient Referral Form. You may also download a copy of our referral form. We ask that you mail, fax (614)526.3745 or have the patient bring the slip with them on their first visit to our office.
