Insurance Information

We realize the financial aspects of healthcare can be confusing. Our staff can help alleviate uncertainty by filing appropriate charges with your primary and secondary insurance companies. 

Please provide us with accurate and current insurance information and proof of coverage (insurance ID card or insurance forms) at your initial consultation. These documents will provide essential information that will help us determine any special requirements in your plan, such as how often you may be treated and who may provide treatment to you/any family members covered by your plan. 

Richard E. Scheetz, Jr., DDS, MS is also an in-network provider with Delta Dental Premier. Gregory A. Rekos, DDS, MS is an in-network provider with Dental Premier and Anthem 100, 200, 300. Please take time to review the following checklist to help you provide us all of the necessary information at your first appointment. 
 
If you have health or dental insurance: 

  • Determine whether or not Oral & Facial Surgeons of Ohio is a participating provider under your insurance plan.
  • If Oral & Facial Surgeons of Ohio is not part of your network, you may still have the option to use out-of-network benefits. The insurance company can give coverage percentages for both in-network and out-of-network providers.
  • Some medical insurance companies cover impacted wisdom teeth (complete bony/partial bony). Aetna Medical, Medical Mutual Medical, and specific UHC Medical plans are examples of insurance that may cover impacted wisdom teeth depending on your employer group. It is advisable to call about your medical insurance plan to see what, if any coverage you have for wisdom teeth removal.
  • As a courtesy, we will submit the initial claim to your insurance company for services we provide you.

You will be responsible for all charges your insurance company does not cover due to unpaid deductibles, coinsurance, or non-covered services.

If you do not have insurance coverage or if non-covered services are provided, please call our office to obtain a written estimate of charges for the services you plan to have done at your appointment. This allows us to discuss the deposit required for your type of procedure.
