Patient Registration

You may preregister with our office by filling out our secure online Patient Registration Form. You can fill out the form online and print to bring into the office at the time of the appointment. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

In order to make your visit as smooth as possible, we would appreciate completion of the accompanying forms. Please bring these completed forms with you. This will allow us to treat you in the safest manner, and also make your visit as short as possible. If you have any information from your dentist including x-rays and referral slips, please make sure to bring them with you. Also, we would appreciate your insurance cards, both medical and dental, which will help us in your insurance processing.

If you are going to have surgery on your first visit, it is very important that you have nothing to eat or drink after midnight. This is especially true if you plan on having sedation or general anesthesia, as a medical emergency can arise if you have something to eat or drink 8 hours or less before your surgery. If you do take medications, you can take them safely with a sip of water. It is strongly suggested that you take all of your normal medications in the morning of your surgery. Please try to wear a short sleeve shirt, and loose clothing, as this will allow us to place the monitors so that we may safely monitor you during your surgery. Someone will need to transport you here and to home after surgery if you plan on having sedation or general anesthesia. Please do not wear earrings and prepare to remove or not wear your contacts.

It is our office policy to collect all consultation and x-ray fees at time of service. It is also our office policy to collect 35%, if insurance coverage applies (100% if no insurance benefits) of the cost of surgery the day of the surgical appointment. This often covers any deductible or co-payments for most insurance carriers, however, there may be a balance left after insurance has paid. We offer affordable low interest financing through Dental Fee Plan to anyone who would like to make low monthly payments. If you are concerned about any health problems, finances, or have significant questions, we suggest that you make a consultation appointment so that these may be discussed in detail prior to your surgical treatment.
